The Rev. Martin Madan, M.A. (1726-1790) was a Church of England clergyman and itinerant Methodist preacher of Calvinist persuasion. This text is found in the hymn collection he published in 1769. It is found set to this tune in Samuel Worcester's Psalms, Hymns & Spiritual Songs of the Rev. Isaac Watts, D.D., to which are added Select Hymns from Other Authors (Boston, 1824). I have added the alto and tenor parts since only the treble and bass is found in the original.
